Terms and conditions

  1. All entries must include name, address and phone number. Proof of posting is not proof of delivery.
  2. There are 4 prizes
  3. Prize draw will take place on September 30, 2008. There will be 1 winner for each prize.
  4. Winners will be drawn at random and will be notified within 1 week of the prize draw by phone.
  5. The draw is open to London Borough of Newham residents. One entry per person. Entries received from third parties or agents will not be entered into the prize draw.
  6. The prize is subject to availability. Newham PCT reserves the right to substitute a prize of equivalent value where necessary for reasons beyond its control.
  7. No cash alternative will be offered for any prize.
  8. A list of winners may be obtained by written request by sending an SAE to Newham Health Debate, Newham Primary Care Trust, 2 Western Gateway, Royal Victoria Dock, E16 1DR by 30 November 2008.
  9. Newham PCT’s decision is final and no correspondence will be entered into. Entries that do not comply may be disqualified.
  10. Newham PCT reserves the right to alter, amend or foreclose this Free Prize Draw without prior notice if unforeseen circumstances make this unavoidable. The promoter is Newham Primary Care Trust, which is an NHS Trust and public service.
